Customized Care Plans
Every individual has unique care requirements. Our Health Care Professionals work closely with you and your family to create a personalized care plan that ensures you receive the support you need, when you need it. We are committed to providing compassionate care that aligns with your preferences and values.
The Process
The process begins with a referral.
A referral which can be a self referral or come from a social worker from a hospital, nursing facility, Area Agency on Aging, etc.

Once referral is received, Administrator will set up an intake visit at no charge. during intake visit information is gathered & shared. Schedule is discussed. From that time it takes approximately 5-10 days to begin services.

Insurance and Payment
For non medical home care, long term care insurance is accepted. Funding is available through local Area Agency on Aging if eligible.

Billing is done every other week. We take payment by check, credit card or automatic withdrawal from checking account. We require a 4 week deposit.
Our Team
All staff are screened through PA state police, FBI background check, National sex offender & we do random drug screenings.

Some of our staff is experienced in wound care, hospice care, Alzheimer's care, and/or trained in suicide prevention. We also have bilingual staff members.
Our Story
Sinclair's Home Care was founded in 2013.
I had been working in home health since 1994. When my own family member became ill, I saw how difficult it was to obtain quality home health care at a cost that was affordable.
My goal was to keep my family member at home. I knew this was something many families wished they could do.
Drawing on my experience, I took on my first client in 2013 & brought in a couple local caregivers to assist me.
In the 10 years, Sinclair's has grown & now serves multiple counties with 50 plus caregivers.
Sinclair's continues to be solely owned & operated by Jennifer Sinclair-Moore.
